By checking the box below I agree that I understand that the movement-based practices offered during the Ridiculously Alive Retreat are devised to make the body strong and flexible. Yoga and movement-based practices affect the body’s glandular secretions, muscles, nerves, circulation and digestion. I realize that it is important never to do any practice to the point of pain or discomfort. I am aware that there is some risk involved in all physical exercise and I am responsible to recognize my own physical limits. I certify that I am in good health. If I am under the care of a physician, he/she is aware of and approves of my participation in Yoga instruction. I certify that I am not attending the Ridiculously Alive Retreat for the treatment of any physical infirmity or chronic ailment or injury. I am aware that Yoga may help manage and strengthen my general health and vital energy. In consideration of participating in the practices and activites of the Ridiculously Alive Retreat, I agree that I (nor my heirs, guardians, legal representatives and assigns) will not make a claim or file an action against the Megan Lipsett or Summer Shapiro for injury or damages resulting from claims of negligence howsoever caused, in connection with the activities facilitated at the Ridiculously Alive Retreat. In addition, I hereby waive, release and discharge Megan Lipsett and Summer Shapiro from all actions, claims or demands I, my heirs, guardians, legal representatives or assigns now have, or may hereafter have, for injury or damages resulting from my participation in the Ridiculously Alive Retreat. I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A WAIVER AND RELEASE OF POTENTIAL LIABILITY AND A CONTRACT BETWEEN MYSELF MEGAN LIPSETT AND SUMMER SHAPIRO, AND SIGN IT OF MY OWN FREE WILL *
